Would someone be willing to give me a quick course in Burner Phones 101?

How are they purchased?

Do they require some online account to be opened?

Do they come with a phone number?

Are calls made on a burner phone tracked/logged somewhere?

Are extra minutes purchased online or in stores or...?

Any help would really be appreciated, and no info is too easy/simple!
