A quick search turned up a possible answer: ghosted. Is that it?
As an aside, I'm one of those that has trouble with names - people, places, things, it doesn't matter, if it's a name, it will just flit right out of my head at times.
When this happens, especially for a person's name, I have a trick I use to help me remember. I start going through the alphabet, one letter at a time, and see if I think the word I'm looking for begins with that letter. Usually, when I hit the right letter, I know it, and if I start cycling through names/words that start with that letter, I come up with what I'm looking for.

I emailed a guy I used to work with, he sent a Facebook message to one of the UX guys and he sent us the word.
