OK since we are guessing - I'm thinking they are biological siblings. The original title was something like "Call of Blood" or something like that. Makes me think they are bio-related.

Well guess what? I was downtown last night at an improv comedy place, and I saw a poster for 'Hearts Full of Blood' that looked exactly like this:

One review said it was "Based on a true story," the program cover insists, and yes, the story is about as dark as one can envision without entering Sweeney Todd territory."
Another calls the twist, which centers on the female lead "Truly, horrifically, tragically, disgustingly shocking."
Based on this, I am guessing serial killer.

I will prob never get to see this so I want a spoiler too! I think the sibling thing is the most shocking...also...using logic, a reason for many miscarriages!

Found this.
It's difficult to say much about the play's plot without giving too much away, but this twistedly funny play mixes the pathos of Oedipal tragedy with ripped-from-the-headlines sensationalism. Imagine, if you will, what Oedipus would look like on Dr. Phil and you've got some sense of the impetus behind Hearts Full of Blood.
Your mileage may vary... in accordance with the prophecy.

The only conclusion I am able to draw from the lack of spoiler in this thread is that PPGA actually hates us and wants us to suffer.
